SUMMARY:Community Conversations: Living well with long-term conditions
DESCRIPTION:Over 48% of Canberrans live with a long-term health condition\, but many of us don’t access supports that can help us manage our health. With many of us living longer and hospitals under strain\, it’s important to learn about what you can do to stay well and live the best life you can. \nAt this event\, you will hear from 6 allied health services and community organisations about common long-term health conditions\, what support is available to you closer to home and what steps you can take to live better – at home\, at work\, and as you age. \nEveryone is welcome\, whether you are looking for information for you\, your family or people you support. \nHow it will run: \nThe audience will be seated at tables of 6-8 people with food and drinks\, and each speaker will rotate around the room to provide a 5-7min presentation\, and answer questions on their service\, for each table. \nYou’ll hear from… \nCanberra Health Services’ Take Control – Live Well Program\nThe Take Control – Live Well program is a 3-week group program that gives you the skills and confidence to take control of your chronic condition.
